The Webster Chamber of Commerce makes health care and dental insurance available to its business members.

The forms provided here are for the convenience of our members, but you still need to call the office to ensure that all required forms have been completed and received by us.

The plan outlines below contain plans that are currently offered as well as plans that are closed for new enrollment. We still provide the benefits comparsion on closed plans for the convenience of our customers still enrolled in these plans. Please note that the following plans are no longer accepting new enrollment: Blue EPO Balanced 6, Blue Choice Select & Blue Choice Value.

MVP PLANS

The file below is a summary of all available MVP Plans. MVP now defines plans by plan numbers, rather than the plan names that were previously used by Preferred Care. The 2010 Quarterly Insurance Rates document above indicates which numbered plans replace the old named plans.
